What Documentation Is Required for Mental Health or Chronic Medical Conditions Appeals?


When an appeal is based on medical or mental health circumstances, a letter from a licensed provider is required and must include:

  • Confirmation of the condition 
  • Dates of treatment or services 
  • Confirmation that a treatment or management plan is working effectively 
  • The provider’s assessment of whether you can meet SAP probation or academic plan requirements 

If documentation indicates you are not ready to meet requirements, the appeal will be denied. It may be better to wait to do a SAP appeal until you are ready to be successful.
