What Documentation Is Required for Non-Medical Circumstances?
For SAP appeals that are not based on medical or mental health circumstances, documentation must still come from a third party and clearly show what happened, when it happened, and that the issue is resolved or being effectively managed.
Examples include:
- Death of an immediate family member:
An obituary or death certificate - Accident or injury:
Medical bills or provider notes that include dates of service - Personal crisis:
A letter from a trusted authority (such as a Bishop, Dean of Students, apartment manager, or other appropriate authority) that:
- Describes the situation
- Confirms the crisis has been resolved
- Explains why you are now able to meet academic requirements
- Military service:
Official military orders or documentation showing dates of service
