What Documentation Is Required for Non-Medical Circumstances?


For SAP appeals that are not based on medical or mental health circumstances, documentation must still come from a third party and clearly show what happened, when it happened, and that the issue is resolved or being effectively managed.

Examples include:

  • Death of an immediate family member: 
    An obituary or death certificate 
  • Accident or injury: 
    Medical bills or provider notes that include dates of service 
  • Personal crisis: 
    A letter from a trusted authority (such as a Bishop, Dean of Students, apartment manager, or other appropriate authority) that: 
    • Describes the situation 
    • Confirms the crisis has been resolved 
    • Explains why you are now able to meet academic requirements 
  • Military service: 
    Official military orders or documentation showing dates of service
