A complete participant list including chaperones and non-students with their emergency contact information is required at least 3 days prior to departure. The list may be attached to the trip Spend Authorization in Workday or emailed to traveladmin@byui.edu
Faculty members taking students on required academic field trips must provide participating students with copies of the Authorized Absence Form at least two weeks in advance of the trip so students may obtain signatures from faculty whose classes they will miss. The signed form will then be submitted to the sponsoring faculty member to verify that the student has made appropriate arrangements in his or her coursework.
Waiver for Student Travel and Activities
An activity waiver should be used for all non-required student tours and travel or for required travel if the students opt to make their own travel arrangements. Contact Travel Administration for a link to the electronic activity waiver set up for your specific trip. (traveladmin@byui.edu or ext.1975) Waivers will be retained by Travel Administration for two (2) years plus the current year unless an incident occurs, then they should be retained for five (5) years plus the current year.
This electronic deviation form should be used in situations where a travel participant requests to deviate from the travel group's planned itinerary.
