Important Information to Know:
- Only full-time benefited employees and their spouses can receive premium refunds.
- Retirees over the age of 65 can participate but do not qualify for the premium refund. Early retires under the age of 65 qualify for the refund until they’re 65.
- You can register at any time for the Living Healthy program once you have your DMBA account set up by clicking on the Living Healthy tab on your dashboard on DMBA’s website.
- Each wellness challenge must be completed with 75% effort and be coupled with 150 minutes of exercise each week to qualify for the premium refund.
How to Receive Premium Refunds:
Earn $30 for each of the 6 Wellness Challenges you complete in a year for a total value of $180. DMBA has a list of challenges you can choose from or you can make your own! Once you have picked your goal, you will report your completion every 2 months. By clicking the link below, you can choose which challenge is right for you! There will also be the start and end dates as well as the reporting window of completion for each challenge.
You and your spouse can also earn $70 each by completing a Health Risk Assessment (HRA) at the Student Health Center. You can complete an HRA anytime throughout the year. Here’s how:
- Call the Student Health Center at 208-496-9300 and make an appointment—appointments are available Monday–Friday, 8 a.m.–10 a.m.
- Fast and bring your DMBA insurance info to your appointment
- Get your blood drawn for the test
- Submit your completed assessment to DMBA

2. Another option for reporting is simply to email If you choose to report via email, you will need to include:
- What challenges you completed
- Who completed the challenge (you and/or your spouse)
- Your work email or DMBA ID number.
If you are past the deadline and are reporting a completed challenge late, you will also need to email DMBA’s Wellness Customer Service and include:
- Your name
- DMBA ID number, stating which challenge you completed, and
- When but that you did not report it on time.
Lost/Forgotten DMBA ID Number:
Your DMBA ID number is 9 digits and can be found on the back of your insurance card, starting with 2 or 3 zeros’. If you have lost your card and/or don’t know your number, you will also need to email: WellnessCustService@dmba.com
Once you have been verified, DMBA will assist you with your ID number and get you a new insurance card.
