DMBA offers four medical plans to BYU-Idaho employees: PPO90, PPO70, HSA80, and HSA60. Each plan provides different levels of coverage.. Both HSA80 and HSA60 plans are Health Savings Account compatible plans and have an accompanying employee HSA contribution.
DMBA provides a network of physicians and facilities throughout Utah and southeast Idaho. Preventive services are covered at 100% when done with an in-network provider. In-Network deductibles only apply to HSA80 and HSA60. Depending on the procedure, some services must be pre-authorized by your In-Network Primary Care Physician (PCP).

- Occupational Accidental Death and Dismemberment insurance which is only offered to the employee, not spouse or dependents. (Up to $100,000)

The Supplemental Life Insurance plan is in addition to the basic life insurance amounts included in the monthly medical premiums.
You can change or apply for supplemental life insurance (SGTL) at any time. It is not constrained to the open enrollment period. Upon enrollment, a questionnaire will be provided for each individual who will be included in supplemental coverage. From that point, DMBA might contact you to schedule a nurse's visit to get more information and samples. Coverage does not take effect until DMBA approves the application.
24-Hour Accidental Death and Dismemberment Insurance (24-hr AD&D) provides protection for accidents that result in the loss (or loss of use) of the following: life, limb, vision, hearing, and/or voice. To collect this benefit, the loss must be the direct result of an accidental injury, being independent of all causes other than the accident.
There are several coverage options, including employee only, employee and spouse only, employee and children only, or entire family coverage.
