A signed Activity Waiver is necessary for all non-required student tours and travel as well as for required travel, if a student chooses to make their own travel arrangements. An employee should complete the top portion paying special attention to the specific risks associated with the activity. Signed waivers may be uploaded into the Spend Authorization in Workday or emailed to email@example.com. Waivers will be retained at the Travel Administration Office for two (2) years plus the current year unless an incident occurs then they will be retained five (5) years plus the current year.
