Step 1: Fill out the Application

• Provide the most recent documentation from a licensed medical professional that clearly states your diagnosis. Individual Education Programs (IEPs) or 504 plans are also accepted.

Step 3: Wait to Be Contacted
• Our office will contact you to schedule an appointment with one of our administrators.

• Accommodations will be set up after your meeting and once your application is reviewed.

Step 4: Communicate with Your Teachers
• We will email a Faculty Notification Letter to each of your teachers.
