Accessibility Services Forms

This form allows students to give permission for us to share information with a parent, teacher, or other individuals. Once signed, the student allows the Accessibility Services Office staff to discuss academic progress or other concerns with the named individual.

This form allows students to authorize a doctor or college to send information to the Accessibility Services Office. Once signed, the student gives permission for the Accessibility Services Office to receive documentation from another source.
