Steps to Receiving Accommodations

Follow the steps below to receive accommodations from the Accessibility Services Office.
Step 1: Submit Your Application and Documentation
· Log in to the Accommodations Login using your BYU-Idaho username and password.
· Upload recent documentation from a licensed medical professional that clearly states your diagnosis. IEPs and 504 plans are also accepted.
· Complete the application form and click “Submit.”

If you're unable to upload your documents online, you may:
· Bring them to our office at 181 Manwaring Center
· Email them to accessibility@byui.edu

Step 2: Complete Consent Releases (If Needed)
· If you would like our office to communicate with doctors, parents, high schools, or other third parties regarding your accommodations or documentation, you will need to complete a Consent to Release Information form.

· These forms are available through the Accommodations Login application.
Step 3: Check Your BYU-Idaho Email for Appointment Information
· After reviewing your application, our office will email you to schedule an appointment with an accessibility administrator.

· Online students will meet with an administrator over the phone or via zoom.

· Accommodations will be set up after your appointment and final review.
Step 4: Communicate with Your Instructors
· After your appointment and your accommodations are approved, you will need to communicate with your instructors.

· We will send a Faculty Notification Letter to each of your instructors stating your approved and requested accommodations.

· You are responsible for meeting with each instructor to discuss your accommodations at the beginning of every semester.
