BYU-Idaho Permit-Required Confined Spaces Entry Form
The following guidelines should be followed when entering a "Permit-Required Confined Space."
[ ]Obtain Permit from the Safety Office or from Campus Police
[ ]Dispatcher logs location, purpose, and duration of entrance
[ ]Evaluate and eliminate potential hazards prior to entrance
[ ]Monitor air quality at entrance and work level prior to entering space
[ ]DO NOT ENTER IF UNSAFE CONDITIONS EXIT
[ ]Utilize appropriate PERSONAL PROTECTIVE EQUIPMENT
[ ]Utilize BODY HARNESS, CABLE, WINCH AND TRIPOD when potential electrical, flammable, atmospheric or engulfment hazards may be generated by work performed
[ ]ATTENDANT REQUIRED for every person entering permit required confined space
[ ]Radio or Cellular phone communications required for attendant
[ ]Enter and perform task in confined space
[ ]Attendant continuously monitors welfare of entrant
[ ]Attendant continuously monitors air quality in confined space
[ ]CALL 911 IF RESCUE IS REQUIRED-- Police will dispatch Fire Department Confined Space Rescue Team
[ ]ATTENDANT DOES NOT ENTER CONFINED SPACE
[ ]External rescue may be attempted by attendant using cable and winch
[ ]When work is completed, confined space is secured
[ ]Dispatcher is notified and air monitor returned to police
[ ]Completed confined space permit is submitted to Safety Office
