1. Start by clicking the Service Request button on the Facilities Management home page:
2. Then select your facility on the following page:
3. Please fill out the Service Request form with as much information as possible. If there isn't a field available for something you want to include, just put it in the "Action Requested & Account Number" field:
4. Check the "Notify Me" box if you would like to receive email updates on this request.
