Responsible for providing tracking services for juveniles on probation, Tracking services are to include; ability to influence clients as a positive role model, and assist in making positive life choices. Will meet with clients at school and home, and will assist clients in meeting restitution and community service goals. At times may be asked to transport clients to and/or from counseling sessions, work detail, and community service destinations. May be asked to administer drag and alcohol tests for female clients and curfew checks at odd hours. May be asked to tutor clients struggling in areas of difficulty regarding schoolwork.

Psychosocial Rehabilitation Specialist

Objective: To teach youth and their families life skills so they can be more happy, healthy, and a positive part of their family. We accomplish this by working with both the youth and their family on building the skills the youth needs in order to succeed.

Responsibilities:

  1. Provide one-on-one time with an assigned youth on a weekly basis.
  2. Work with family members, extended family members, school personnel, and other adults involved with the youth.
  3. Participate in community based activities to teach the child hands-on life skills.
  4. Follow the youth's service plan to reach the goals set for the youth.
  5. Maintain progress notes on daily activities with the youth.
  6. Meet with the youth's clinician to review their progress.
  7. Make and return phone calls related to the youth as needed.
  8. Complete reviews as necessary.
  9. Complete 20 hours of professional development per year.

Educational Requirements: Must hold a bachelor's degree in a human service field such as psychology, social work, or a related field. If an individual does not possess the above degrees, they may have twenty-one (21) college credit hours in human services; these credit hours must be approved by CenterPointe.

Hours: As arranged with assigned family

Pay Range: $14.00 - $18.00/DOE

Starting Salary:    The salary range for this position is $35,000.00 to $42,000.00.

GENERAL PURPOSE

 

Performs a variety of skilled technical and professional level case management duties involving evaluation, supervision, and counseling of juveniles in residential and transitional care program and assists individuals in setting and achieving program and treatment goals.

 

SUPERVISION RECEIVED

 

Works under the general supervision of the Program Manager.

 

SUPERVISION EXERCISED

 

May provide general or close supervision to program officers assigned to the Treatment Program.

 

ESSENTIAL FUNCTIONS

 

Assists the program manager in the implementation, evaluation and development of program and treatment goals for assigned juvenile cases; develops treatment plans; identifies time frame based treatment and goals; coordinates with parents, counselors, state officials, etc.

 

Performs professional case management; supervises assigned juveniles with behavior, mental health, and substance abuse problems in both residential and transitional care and assists them in setting and achieving program and treatment goals.

 

Provides individual and group counseling and therapy for juveniles and their families as directed.

 

Creates initial medication logs, monitors medications and medical appointments for all program residents; initiates changes in medications and obtains medical doctor approval.  

 

Conducts home visits and meetings with juveniles, family members, probation officers, educational officials, and other appropriate individuals or agencies and keeps the program manager and other interested parties informed regarding progress and any significant incidents or changes in status of assigned individuals.

 

Maintains detailed juvenile case files and related records and prepares and submits a variety of written reports to the Department of Juvenile Corrections, the Courts, probation officers and other related officials and agencies as required.

 

Assists in managing program juvenile caseload and transitions through local wrap around services. 

 

Transports juveniles to and from various required appointments, classes, and meetings as needed.

 

Assists the program manager in general program development, implementation and administration including staff development and training.

 

Assists the program manager with preparation of grant applications and required reports.

 

Performs related duties as required.

 

 

MINIMUM QUALIFICATIONS

 

1. Educations and Experience:

 

A.    Bachelors degree in social work, psychology, or a related field;

                                                                                                   AND

B.     One year of related work experience;

OR

C.     An equivalent combination of education, training, or experience.

 

2.   Required Knowledge, Skills, and Abilities:

 

Working knowledge of Idaho juvenile corrections system; legal liabilities associated with juvenile detention operations and drug and alcohol treatment alternatives; generally accepted case management practices for juveniles with behavior and substance abuse problems in court ordered treatment programs; multidisciplinary counseling and treatment methods; substance abuse treatment methods and techniques; individual, group and family counseling methods and techniques; laws and regulations governing juvenile detention programs and facilities; and various community, state and private agencies or organizations involved in treatment and counseling of juveniles; personal computer operations and related software applications.

 

Skill in interpersonal communications including use of proper English grammar, composition, spelling and punctuation, efficient in use of computer/word processing programs and in the application of generally accepted counseling techniques and methods.

 

Ability to identify, implement and manage various and appropriate treatment modalities; communicate effectively, verbally and in writing; develop effective working relationships with juveniles and their families, detention staff, probation officers, professionals, the courts and other agencies; maintain detailed records and prepare a variety complex detailed written reports; ability to exercise independent judgment and apply problem solving methods to analyze complex situations and develop effective treatment plans using a multidisciplinary approach; to be flexible and respond quickly and appropriately under stress in a variety of unusual or unexpected situations; and to effectively use computers and standard office equipment and systems to establish and maintain detailed case records and reports.

 

3.   Special Qualifications:

 

Must be licensed in the State of Idaho as a Social Worker

Must possess and maintain a valid Idaho motor vehicle operator's license.

Must be at least 21 years of age.

First Aid and CPR certification.

No relevant criminal history as confirmed by NCIC and local background check.

Must be available for on call work.

 

4.   Work Environment:

 

Tasks require variety of physical activities, not generally involving muscular strain, however, does involve walking, standing, stooping, lifting, sitting, reaching, talking, hearing and seeing.  Mental application utilizes memory for details, emotional stability, discriminating thinking and problem solving.  Duties are generally performed in a climate-controlled environment with no significant exposure to weather conditions. Job functions involve working with juveniles in secure residential, home and community situations involving some risk of injury.  Local and occasional extended area travel required in course of performing portions of job functions.
