Print a copy of this request.                                                   

After printing a copy of the request, click on the "Submit Request for Approval" button.

The permit request needs to be submitted no sooner than 24 hours before the work is to be performed. 

The permit will be issued by someone from the Safety Office prior to the work. 

If the form is submitted less than 24 hours before work begins, you will need to call 496-2481 and explain the emergency requiring this work.
