Permit Worksheet

The following worksheet must be filled out and turned into the BYU-Idaho Safety Office for approval 48 hours prior to any open flame demonstration.

Open flames and flame producing devices shall be monitored constantly by a trained person.

A minimum of one fire extinguisher, listed for the purpose, shall be located within 30 ft of the area where open flames or flame-producing devices are in use. Number of extinguishers is determined by the Safety office/Fire protection office.

A current material safety data sheet (MSDS) for the materials (fuels) consumed in the flame effect shall be on file in the Hazardous Materials office. Contact Byron Gilbert ext. 2414

Fire safety personnel have been orientated to the means of transmitting an alarm and notifying proper agencies of an emergency.

There shall be no glowing or flaming particles within 10 ft of the audience.

There shall be a divider provided to separate the audience from the area where the flame production is taking place.

Please provide a brief narrative of the event involving flame production or open flame of any kind. Please include the type of show, and the expected duration of the show.

When requested by the Fire Protection Officer, please provide a drawing of the area for the Open Flame Production or Demonstration. Please include where the stage is and where the audience is and where fire safety personnel are located with fire extinguishers.
