The lab is here for your use. If you take good care of the lab and abide by the following policies, you will have a lot of flexibility and freedom to use the lab. Please read each of the following policies carefully, and then date and sign this document. You will be given the combination to the lab after you have signed and turned in this form.
  1. Use of the lab is restricted to the hours that it is officially open.
  2. The BYU-Idaho Dress Standards and Honor Code are in force while using the lab.
  3. Do not abuse any of the equipment in the lab in any manner.
  4. Do not load any software on to any computer in the lab.
  5. Do not reconfigure the computers in any manner (i.e., change screen settings, backgrounds, printer definitions, etc.)
  6. You may copy and save files only to C:\Temp folder. Do not copy, rename or delete any other files on the C: drive.
  7. During the day log off the computer each time you are finished using it. In the evening or on Saturday, shut the computer down and turn the power to the computer and monitor off.
  8. Food and drink are not permitted in the lab.
  9. Report computer problems to a IS faculty member as soon as possible so they can be fixed in a timely manner.
  10. The combination to the door to Smith 422 lab is confidential and should not be shared with anyone else under any circumstance. Open use of this lab is restricted to IS majors enrolled in a class that is taught in this lab. This means friends, spouses, roommates, etc. are not allowed in the lab unless an IS faculty member has given explicit permission. If you see someone in the lab that you think is not a IS major that is not enrolled in one of the approved classes, please challenge the student. Invite them to leave and please report the incident to one of the IS faculty members.
The IS department reserves the right to restrict access to the lab or change the combination of lock on the door to the lab at anytime. Violation of the above policies will result in your loss of lab privileges and may result in further action be taken with student life.

I have read and agree to abide by the above lab policies.
