Application for Child Enrollment

 

 

Please read the following information before completing the application.

 

 

1.      We enroll children ages 3, 4, and 5 in the BYU Idaho Child Labs.  Parents are responsible to read the following enrollment requirements and to assure that their child qualifies for enrollment.

·        Children must be at least 3 years old (36 months) on the first day of school.

·        Children must not have had a fifth birthday before September 1.

·        Children will not be enrolled for two consecutive semesters.  (Summer terms are not considered one of the two major semesters.)

 

2.      Applications should be submitted several months in advance of the desired session.  Enrollment selection is based on the order in which applications are received.  We reserve the right to make selection adjustments as necessary to keep favorable male/female, race/ethnic ratio and age mix in the labs.  In the summer, priority is given to 5 year-olds starting kindergarten in the fall.

 

3.      The main purpose of our program is the training of teachers.  One critical aspect of teacher training is learning how to effectively interact with parents.  At the beginning of each semester, a parent orientation meeting will be conducted by our student staff.  You are required to attend this meeting as a condition of your child’s enrollment.  (You will be notified of the date and time.)  At this meeting, you will be asked to complete and sign the following forms and/or wavers:  consent for observation, testing, photography and video taping; acknowledgment of risk; statement of insurance or assumption of medical expense; and indemnification of BYU Idaho and Madison School District and their employees in the unlikely event of a serious accident.  You must also provide evidence that your child has received all required immunizations.  Please bring a photo copy  of your child’s immunization records to the parent orientation meeting.  Feel free to call and discuss concerns about any of these items with the department secretary at 496-1346 or the lab director at 496-1355.

 

4.      Notification of your child’s acceptance into the preschool will be made by mail at least a month before the lab session begins.  After receiving notification, you must:

a.      Call the department secretary at 496-1346 to confirm your intention to have your child in the lab.  If you do not confirm, you will lose your spot.

b.     Pay the lab fee at least two weeks before the session begins.

 

5.      The lab fee for each child per session is:

                                                   Monday, Wednesday, and Friday          Tuesday and Thursday

              Community Patrons                                  $125.00                                       $85.00

              Full-time BYUI students                            $80.00                                         $55.00

         

This fee schedule reflects tuition as of Fall 2004 and is subject to change.  Fees are to be paid at the Cashier’s Office after your child is accepted into the program.

 

6.      Preschool usually starts about two weeks after the beginning of the new semester.  Student staff will contact you by telephone to tell you which lab your child will be in.  After you have been called, you may arrange to bring your child for a brief visit to the center to get acquainted with the facility and the teachers.
