Mission Physicals
What to do:
The mission physical requires at least three separate appointments; you will need to call the Student Health Center at 208-496-1300 to set up these appointments. Please inform the receptionist that you are making an appointment for a mission physical so they can help you set up those appointments. The first appointment consists of collecting lab work and this appointment may not take place on Thursdays. Please contact the Student Health Center for more information.
What to bring:
- Physician's Health Evaluation Form
- Personal Health History - You must fill this form out online before your appointment and print it off when complete.
If you have not received the password to fill in the electronic forms, return to your Bishop and get the password. We have been directed by the Missionary Department to use the online forms.
- Immunization Record. Bring a copy of the written dates of all the immunizations you have received.
If your parents do not have your immunization records, try your high school or the location you received your immunizations. You may have these records faxed to our office (Fax Number: 208-496-1306). If you need to update or receive any immunizations, you can do that at the Student Health Center. Current Immunization Prices.
- Your bishop's address - We will mail your completed paperwork to him. We will provide the envelope and postage.
Please don't ask to hand carry your mission physical to your bishop. We have been instructed to mail them to your bishop directly.
Preparing for your physical:
- We are required to draw blood and collect a urine sample. Be sure to drink plenty of water before you come.
- Wear clothing that will allow us to easily examine you. Shorter sleeves will allow for the blood draw.
