BYU-Idaho Health Insurance Requirements
Insurance Requirement
- BYU-Idaho requires all continuing students to have adequate medical insurance.
- You will automatically be enrolled in the Student Health Plan when you first enroll for classes.
Waiving Enrollment in the Student Health Plan

You may waive BYU-Idaho Student Health Plan coverage only if you are already covered by your parent's insurance, or by an employer's insurance policy. Waivers must be submitted by the 15th school day of the semester.
- Two ways to waive insurance:
- During the registration process, you may submit the waiver online.
- You may print out the waiver here and submit it to the Student Health Center within 15 school days of the beginning of the semester.
- If your coverage from another insurance plan is discontinued while you are attending BYU-Idaho, contact the Health Center immediately.

Health Plan Exclusions
Exclusions are any charges which do not meet the definition of eligible charges and any charges for services provided outside the Student Health Center for pre-existing conditions for one year (12 months) following the participant's effective date of coverage; or, diagnostic purposes which are not related to an injury or sickness, unless otherwise provided for by the terms of the Plan.

Deductibles
Deductibles only apply to the married student insurance plan. Please note that the deductibles run on the academic calendar year (September 1st to August 31st) not from January to December. See the Health Insurance Manual to find out more information about the deductibles.Privacy Practices

Medical Records
The Student Health Center may furnish copies of patient records when requested with a patient-signed authorization of release. A release form is available at the Health Center, or a written request can be made with the following information:
- Patient name
- Patient social security number
- Patient birth date
- What records are being requested?
- Where are records being sent?
• Name
• Address
• Phone number
• Fax number - Patient signature
- Date
A signed authorization is required each time records are requested.
Due to HIPPA regulations, no medical information can be shared without signed authorization. This includes information to parents and/or a spouse.
