Before clicking on the link below, read the following instructions:

1. After clicking the link, you will be taken to the Online Scheduling system.
   • If you have NEVER registered before, click on "Click here to register" (this is right under Forgot log in/ Password).

    You need to create a username and password in order to log in, please

   create these so that you will remember them, and then log in with your new username and password. (This log in and password can be the same that you use for your BYU-I account.) 

   • If you HAVE registered before, then log in with your username and password.

   If you have forgotten your username and password, click on "Forgot Log in/Password" and it will e-mail this information to you (to the e-mail that you used when registering the first time).

2. After logging in, there is a list of links that are available to you; you are interested in the link that says "Medical History-Idaho Patients Only".
3. Fill out this form and then click "Submit".
