Fill out and Print the following forms after you meet with your bishop:
- Physician’s Health Evaluation
- Personal Health History of Missionary Candidate
- Additional Health History
- Immunization Records
- Bishop's Address
Schedule two appointments at the Student Health Center by calling 208-496-9330:
- Immunizations Nurse and Lab
- Physical appointment with a provider two days later
Remember to prepare for your appointment by:
- Drinking plenty of water
- Wear eyeglasses or contacts if you normally wear them
*The Student Health Center will mail your mission papers to your Bishop once they are complete.
