Nauvoo Student Tours: Registration

 

Register by Mail or In Person

Three forms are required for participation in a Nauvoo Tour. Be sure to print, fill out, and mail all three forms with your payment.

 

  1. Print the Application, Medical Information, and Liability Waiver forms. We do have these forms already printed in our office (103 ASB), and you may come in and just fill them out.
  2. Complete all three forms and mail  or bring into our office. Payment must accompany the forms in order to be registered.
