         Confined Space Permit Instructions    


Read and follow these instructions when completing the Confined Space Work Permit.

  1. Read and comply with the University Confined Space Entry Policy.
  2. All persons involved with confined space work must be properly trained and the training must be documented.
  3. The Permit should be completed by a designated "Competent Person."
  4. Fill in those blanks on the form that can be completed prior to an evaluation of the work site and complete the Required Check List portion of the Permit form by mouse clicking on the appropriate answer to each question.
  5. Use the program tool bar to print a copy of the Permit. (Print the form before clicking on the Submit button).
  6. Click on the "Submit" button at the bottom of the Permit page (An electronic copy of the Permit will immediately be sent to the Safety Office and to the PPLT secretary).
  7. The printed copy of the Permit must be taken to the work site.
  8. At the work site, use the Industrial Scientific Instrument to monitor the air quality in the confined space and write the readings on the Permit form.
  9. If safe, enter the confined space and perform the intended work in compliance with established safety policies and procedures.
  10. Send the fully completed copy of the Permit to the Safety Office to be filed.
