The following reporting procedures have been provided to better serve the
university community as well as help the Safety Office track the number of injuries/illnesses claims,
accidents, and potential safety hazards on campus. This provides an excellent
opportunity for the campus community to become involved in safety and promote awareness.
Your prompt response in filling out the appropriate forms is most appreciated.
Worker's
Compensation Memo: All faculty, staff, and
administrators should review this memo which outlines the procedures to follow when an
employee incurs a work-related injury or illness. All employees, both full-time and
part-time should sign the document and submit it to the BYU-Idaho Safety Office.
Work Related Injury/Illness Report Form:
Any full-time or part-time employee of the university must fill out the Injury/Illness
Report Form for any work-related injury or illness. This form is provided
online.
Incident
Report Form: The Incident Report Form is a
campus-wide initiative to be utilized by all faculty, staff, administrators, and student
employees in reporting all non-work related accidents/incidents occurring on campus
(including: falls, tripping, accidents, etc.). Each department should instruct
their employees on the importance of filling out the Incident Report Form, even if
no serious injury occurs. Your help in tracking incidents will greatly
assist the university in its efforts to improve institutional safety, and is greatly
appreciated.
Hazards
Report Form: The Hazards Report Form is
open to all university faculty, staff, administrators, students, and guests who wish to report
a potential safety hazard. It is now possible to report a safety concern or safety
suggestion online directly to the Safety Office. This is an excellent opportunity for the
campus community to become involved in safety and promote awareness. Types of safety
concerns to report here may include:
1. General workplace and office safety
2. Fire safety
3. Laboratory safety
4. Environmental issues
5. Other
